ONTARIO NATUROPATHIC DOCTORS: LIFELABS® ACCEPTS SPECIMEN REQUISITIONS FOR GENOVA DIAGNOSTIC TESTING
If you are not registered with Lifelabs, you will need to contact them toll free at 844 815-0022. To initiate the process, LifeLabs will provide you with a registration form to complete. Alternatively, the required form is available online at this link.
After you are successfully registered, all you will be required to do is provide your patient with the appropriate Genova Diagnostics test kit and completed LifeLabs requisition form. LifeLabs will invoice your clinic for the sample collection and processing and ship the specimen as per the collection kit instructions.

*Note: Please ensure all Genova testing you order is considered an allowable test based on the July 1, 2015 changes that were made to the Naturopathy Act 2007 and the Laboratory and Specimen Collection Centre Licensing Act and its Regulations.
